Does the guest need an app to scan the code?

No. The phone camera opens the menu straight in the browser, and the guest orders without creating an account.

Can guests pay from their phone?

Yes, when online payment is enabled. The tab can also stay open and be paid at the cashier at the end of the sitting.

What if the menu changes?

The code is permanent and points at your store, so any change to items or prices shows to guests immediately without reprinting.
